Area group makes list of 2018 Healthy Change Champions

The North East Local Health Integration Network is out with its yearly list of Healthy Change Champions and this year seven groups and people are recognized on the year-end list.

One of the groups was recognized just a short time ago and it’s the Nipissing Collaborative Implementation Team.

This group of Nipissing residents developed a navigation tool that helps people with mental health and addictions issues better connect with the services they need.

The navigation tool also helps people with developmental disabilities.

The LHIN says the team has helped to not only improve the mental health and addictions system in Nipissing, but it’s also improved what’s currently available.

The three-year program is in the final stages of a testing phase before it will be used by other service providers in the district.

The team has high hopes for the navigation tool and says there’s no reason why it can’t be adopted by other service agencies in Ontario.

The LHIN compiles its year-end list to showcase the individuals who have improved patients’ experiences across the northeast.
